RESUMEN Introducción: la seguridad durante los procedimientos médicos o quirúrgicos se ve amenazada por la anafilaxia. Objetivo: evaluar el cuadro clínico, método de estudio y eventual resultado de la reexposición en pacientes con reacciones anafilácticas y alérgicas durante procedimientos médicos o quirúrgicos. Materiales y métodos: se realizó un estudio cuantitativo, observacional y descriptivo en 11 pacientes con diagnóstico de anafilaxia en el curso de una intervención quirúrgica o proceder médico, en los hospitales Militar Principal y Nacional Simão Méndes, de Guinea Bissau, entre 2018 y 2020. Se evaluaron las variables: edad, sexo, procedimiento, método anestésico, antecedentes alérgicos, gravedad de la anafilaxia y agente causal. Resultados: de los 11 pacientes estudiados, 10 tuvieron reacciones en procedimientos quirúrgicos. La media de edad fue de 46,09 años, con una relación por sexo de 0,57 casos masculinos por cada caso femenino. Cinco pacientes (45,45 %) desarrollaron el grado máximo de severidad de la anafilaxia. Los agentes anestésicos fueron la causa de la anafilaxia en dos de los cuatro pacientes sometidos al método de anestesia general y en dos de los cinco sometidos al método de anestesia regional. Por su parte, el látex resultó ser el agente causal en el 45,45 % de los pacientes. Conclusiones: la apropiada identificación de las reacciones anafilácticas durante su aparición, constituye la piedra angular de la actuación médica. En cualquier caso, conllevará estudios alergoanestésicos posteriores que definan el agente causal.


ABSTRACT Introduction: safety during medical or surgical procedures is threatened by anaphylaxis. Objective: to evaluate the clinical characteristics, study method and eventual outcome of re-exposure in patients with anaphylactic and allergic reactions during medical or surgical procedures. Materials and methods: a quantitative, observational and descriptive study was carried out in 11 patients with a diagnosis of anaphylaxis in the course of a surgical intervention or medical procedure, in the Main Military Hospital and the National Hospital Simão Méndes, of Guinea Bissau between 2018 and 2020. The evaluated variables were: age, sex, procedure, anesthetic method, allergic history, severity of anaphylaxis and causal agent. Results: of the 11 patients studied, 10 had reactions in surgical procedures. The mean age was 46.09 years with a sex ratio of 0.57 male cases for each female case. Five patients (45.45 %) developed the maximum degree of anaphylaxis severity. Anesthetic agents were the cause of anaphylaxis in two of the four patients who underwent the general anesthesia method, and in two of the five who underwent the regional anesthesia method. Latex was found to be the causal agent in 45.45 % of the patients. Conclusions: the proper identification of anaphylactic reactions during their appearance is the cornerstone of medical action. In any case, subsequent allergo-anesthetic studies would be needed to define the causative agent.

RESUMEN Fundamento: La proteína C reactiva es uno de los mejores marcadores para la valoración y seguimiento de enfermedades inflamatorias; los valores de referencias recomendados para su concentración en suero no están ajustados según género. Objetivo: Determinar si la concentración de proteína C reactiva difiere según el género. Metodología: Se realizó un estudio exploratorio en 3199 muestras de pacientes procedentes del Hospital General Provincial Camilo Cienfuegos y en un grupo control de 76 muestras de sueros de donantes de sangre para cada género, procedentes del Banco de Sangre de Sancti Spíritus. Los niveles se midieron a través un ensayo semicuantitativo de aglutinación con partículas de látex para la muestra supuestamente enferma y uno cuantitativo inmunoturbidimétrico para la supuestamente sana. Se compararon los niveles medios entre género en cada una mediante la prueba t de Student para muestras independientes. Resultados: La media de los niveles de proteína C reactiva en el género masculino y femenino de la muestra supuestamente enferma fue de 3.49 mg/L y 3.41 mg/L respectivamente. En el grupo control la comparación de medias de los niveles de proteína C reactiva entre género fue para los hombres de 1.38 mg/L y para las mujeres de 1.94 mg/L. Conclusión: No se encontraron diferencias significativas entre género en la muestra supuestamente enferma, ni en el grupo control.


ABSTRACT Background: C-reactive protein is one of the best marker for the assessment and monitoring of inflammatory diseases; the recommended reference values for its serum concentration are not gender-adjusted. Objective: To determine whether C-reactive protein concentration differs by gender. Methodology: An exploratory study was conducted on 3199 patient samples from Camilo Cienfuegos General Provincial Hospital and a control group of 76 serum samples from blood donors of each gender from the Sancti Spíritus Blood Bank. Levels were measured by a semi-quantitative latex particle agglutination assay for the presumed diseased sample and a quantitative immunoturbidimetric assay for the presumed healthy sample. Mean levels were compared between genders in each using Student's t-test for independent samples. Results: The mean of C-reactive protein levels in the male and female gender of the supposedly diseased sample was 3.49 mg/L and 3.41 mg/L respectively. In the control group the mean comparison of C-reactive protein levels between genders was 1.38 mg/L for males and 1.94 mg/L for females. Conclusion: No significant gender differences were found in the presumed ill sample, nor in the control group.

ABSTRACT

Contact urticaria is a kind of chronic inducible urticaria, and can be classified into immunologic contact urticaria, non-immunological contact urticaria and contact urticaria of unknown pathogenesis according to pathophysiological mechanisms. The allergens responsible for contact urticaria are various, and there have been increasing reports on contact urticaria induced by related ingredients in cosmetics and by occupational exposure of healthcare workers in recent years. Detailed medical history and physical examination are the key to the diagnosis of contact urticaria, and a variety of skin tests can assist in the diagnosis. The main aim of treatment is to control symptoms, and avoiding contact with allergens is the key to preventing recurrence.

Abstract Introduction Personal protective equipment is mandatory to protect patients and professionals from diseases, especially in the dental environment. The risk of gloves micro-perforations is imminent when using sharp instruments or cleaning them up during lengthy clinical procedures. Objective This study evaluated the integrity of sterile and non-sterile gloves before clinical use and clarified whether friction with disinfectant solution modifies surface morphology and integrity. Material and method Samples of gloves from four different brands were divided into two groups: (1) Sterile surgical gloves (n=260) and (2) Non-sterile gloves (n=260). They were scissored and placed in Ostby's arch so that three solutions - distilled water, ethanol 70°, ethanol 96° - were rubbed with a cotton swab. After 30s, 5, 10, and 15 minutes of solution rubbing, samples were verified by a Scanning Electron Microscope. The pore sizes were measured by Image J software. Result Regardless of the brands, all gloves have been significantly affected by solutions and assessment periods. In general, remarkable changes were evident with ethanol 70° and 96°, and higher pore diameters were observed compared to distilled water. Conclusion Rubbing disinfectant solutions increases gloves' pores sizes, and time negatively influenced its quality.


Resumo Introdução Para proteger pacientes e profissionais de doenças, o uso de equipamentos de proteção individual é obrigatório, principalmente no ambiente odontológico. O risco de microperfurações das luvas é iminente ao usar instrumentos cortantes ou na tentativa de limpar as luvas durante longos procedimentos clínicos. Objetivo Este estudo avaliou a integridade das luvas cirúrgicas e de procedimento antes do uso clínico e esclareceu se o atrito com a solução desinfetante modifica a morfologia e integridade da superfície. Material e método Amostras de luvas de quatro marcas diferentes foram divididas em dois grupos: (1) Luvas cirúrgicas (n = 260) e (2) Luvas descartáveis não estéreis (n = 260). As luvas foram cortadas e colocadas em arco de Ostby, de modo que três soluções - água destilada, etanol 70 °, etanol 96 ° foram esfregadas com um cotonete. Após 30s, 5, 10 e 15 minutos de fricção das soluções, as amostras foram verificadas utilizando um microscópio eletrônico de varredura. Os tamanhos dos poros foram medidos pelo software Image J. Resultado Independentemente das marcas, todas as luvas foram significativamente afetadas por soluções e períodos de avaliação. Em geral, maiores alterações foram evidenciadas com o uso do etanol 70° e 96°, e maiores diâmetros dos poros foram observados quando comparados à água destilada. Conclusão Esfregar soluções desinfetantes aumenta o tamanho dos poros das luvas e o tempo influenciou negativamente sua qualidade.

Introducción. Publicaciones internacionales estiman una prevalencia de sensibilización al látex (SL) en el personal de salud del 7 % al 17 %, y se desconocen los valores en la Argentina.Objetivos. Estimar la prevalencia de sensibilización y alergia al látex en médicos residentes de un hospital pediátrico mediante la prueba epicutánea de lectura inmediata y evaluar factores de riesgo asociados en dicha población.Población y métodos. Estudio de corte transversal. Se incluyeron los residentes, jefes e instructores de Pediatría, Ortopedia, Cirugía y Terapia Intensiva entre junio y octubre de 2017. En todos, se realizó un cuestionario (que evaluó enfermedades atópicas y otros factores de riesgo) y la prueba epicutánea de lectura inmediata. En un subgrupo (residentes de 1ero, 4to año, especialidades quirúrgicas y terapia) se dosó inmunoglobulina E total y específica para látex.Resultados. Se incluyeron 113 participantes. La prevalencia de SL fue del 7,96 % (intervalo de confianza del 95 %: 3,70-14,58); 4 participantes resultaron alérgicos al látex. El antecedente de síntomas relacionados con el látex se asoció significativamente con prueba epicutánea de lectura inmediata + (p = 0,0196; odds ratio 6,13; intervalo de confianza del 95 %: 1,44-26,04). No hubo asociación entre SL y año de residencia.Conclusiones. La prevalencia de SL hallada fue del 7,9 %. Se evidenció una relación significativa entre el antecedente de SRL y un resultado de prueba epicutánea de lectura inmediata positiva


Introduction. International publications estimate a 7 %-17 % latex sensitization (LS) prevalence among health care workers, but values in Argentina are unknown.Objectives. To estimate the prevalence of latex sensitization and allergy among residents of a children's hospital using the immediate-reading prick test and to assess associated risk factors in this population.Population and methods. Cross-sectional study. Residents, trainers, and Chief residents of the Departments of Pediatrics, Orthopedics, Surgery and Intensive Care were included between June and October 2017. All of them were administered a questionnaire (assessing atopic diseases and other risk factors) and underwent the immediate-reading prick test. Total and latex-specific immunoglobulin E levels were determined in a subgroup of individuals (first- and fourth-year residents, surgical specialties, and intensive care).Results. A total of 113 participants were included. LS prevalence was 7.96 % (95 % confidence interval: 3.70-14.58); 4 participants were allergic to latex. A history of latex-related symptoms (LRS) was significantly associated with a positive result in the immediate-reading prick test (p = 0.0196; odds ratio: 6.13; 95 % confidence interval: 1.44-26.04). There was no association between LS and the year of the residency program.Conclusions. The observed LS prevalence was 7.9 %. Abstract Cryptococcosis is a fungal disease affecting more than one million people per yearworldwide. Its main etiological agents are Cryptococcus neoformans species complex and Cryp-tococcus gattii species complex. Cryptococcal meningitis (CM) is considered an AIDS-definingcondition. Rapid diagnosis by cryptococcal antigen assays, either the latex agglutination test(LA) or the lateral flow assay, is key to decreasing mortality due to cryptococcal disease. Theaim of the study was to develop a latex agglutination reagent (LA-ANLIS) for the rapid and reliable diagnosis of cryptococcosis in Argentina. This reagent will be produced in order to supplythe NMLN (National Mycology Laboratory Network). The evaluation of LA-ANLIS performanceand its comparison with the Cryptococcus Antigen Latex Agglutination Test System (LA-IMMY)(Immuno-Mycologics, Inc., USA) were conducted in 94 samples of cerebrospinal fluid. LA-ANLISand LA-IMMY compared exhibited 100% positive agreement and 97% negative agreement. LA-ANLIS showed 94% sensitivity and 97% specificity with the positive and negative predictivevalues of 94% and 97%, respectively. The LA-ANLIS is a reliable, reproducible and cost-effectivereagent, especially useful in countries where the commercial kit is not generally available andmust be obtained at a high cost. National production of reagents is the best choice for a reliableaccess to the rapid diagnosis of CM in Argentina.


Resumen La criptococosis es una enfermedad fúngica que afecta a más de un millón de personas por año en todo el mundo. Los principales agentes etiológicos pertenecen a los complejos de especies Cryptococcus neoformans y Cryptococcus gattii. La criptococosis meníngea (CM) se considera una enfermedad marcadora de sida. El diagnóstico rápido de esta enfermedad a través de la detección del antígeno de Cryptococcus, ya sea por aglutinación en partículas de látex o por inmunocromatografía, es clave para disminuir la mortalidad. El objetivo del presente estudio fue desarrollar un reactivo de aglutinación en partículas de látex para el diagnóstico rápido y certero de la CM en Argentina. Este reactivo (denominado en adelante LA-ANLIS) será producido para abastecer a la Red Nacional de Laboratorios de Micología. Se evaluó el desempeno del reactivo LA-ANLIS, y se realizó una comparación con el reactivo comercial Immuno-Mycologics, Inc. (en adelante, LA-IMMY) utilizando 94 muestras de líquido cefalorraquídeo. Hubo un 100% de acuerdo positivo y un 97% de acuerdo negativo entre los resultados obtenidos con los reactivos LA-ANLIS y LA-IMMY. El reactivo LA-ANLIS mostró una sensibilidad del 94% y una especificidad del 97%; los valores predictivos positivo y negativo fueron del 94 y del 97%, respectivamente. Se concluye que el LA-ANLIS es un reactivo confiable y rentable, que arroja resultados reproducibles, por lo que es especialmente útil en países donde los reactivos comerciales generalmente no están disponibles o sus costos son elevados. La producción nacional de reactivos es la mejor opción para asegurar el acceso de todos los hospitales al diagnóstico rápido de la CM en Argentina.

Abstract Background Dermabrasion is related with mechanical and surgical traumas on the skin; usually topical antiseptics and/or saline have been used for healing. Natural products for wound healing can also be used for abrasions, such as latex from Hevea brasiliensis. Objective This study aimed to evaluate the in vitro viability and migratory/proliferative effects of latex serum from H. brasiliensis and to compare with a commercially available standard antiseptic solution and saline in experimental dermabrasion on rats. Methods For in vitro evaluation, MTT and scratch assays were used. In vivo testing was performed in 72 rats submitted to dermabrasion, treated with saline, antiseptic, or latex serum. This study evaluated re-epithelialization, neutrophilic infiltration, and the quantification of crust and epidermis. Results Latex showed viability at 1% and 0.1% concentrations and migratory/proliferative activity at 0.01% concentrations. The re-epithelialization was highest in latex group on 7th day. The latex group displayed lower thickness of crusts and greater extent of epidermal layers. The latex and antiseptic groups showed increases of myeloperoxidase levels on the 2nd day and showed important reductions from the 7th day. Study limitations Acute superficial wound model in rats and non-use of gel-cream (medium) without latex. Conclusion In conclusion, non-toxic latex stimulated migration/proliferation of keratinocytes in vitro and significantly accelerated wound healing in animal excoriation models compared to chlorhexidine or saline.

ABSTRACT

Formic acid is easily available in open market, because of its wide use in the rubber industry for processing of latex. Because of the easy availability, case of accidental and suicidal poisoning by formic acid are on the rise and most of them end fatally. On reviewing the literature, only a few were found to be reported and studied in detail. MethodsThis study was carried out on seventy-five medicolegal cases, brought with history of formic acid poisoning which were brought for autopsy during the period February 2012 - July 2013. Bits of tissues from upper part of oesophagus, lesser curvature of stomach, lungs, liver, kidney and pancreas were collected. The tissues were processed, and slides were prepared. Microscopic study of each slide was done. ResultsMales (57.3%) outnumbered females and maximum number of cases was of age group 41-70 yrs. (72%). Most of the victims were taken to hospital for treatment and died after admission (85.3%). Laboratory investigations revealed that most of the cases (85.3%) showed impairment in renal function tests, evidence of haemolysis, and metabolic acidosis, corrosion of the upper part of alimentary tract, glottic oedema and brownish fluid in peritoneal cavity. ConclusionsIn Kerala the incidence of formic acid poisoning is on the rise, probably because of the easy availability of the acid to people engaged in rubber industry which is widely prevalent in this state. We make recommendations on product technical requirements or standards to strengthen the quality supervision of products.

Abstract This study aimed to evaluate the anthelmintic and ultrastructural effects of Calotropis procera latex on Haemonchus contortus. C. procera latex was twice centrifuged at 10,000×g and dialyzed to obtain a fraction rich in proteins, named LP (latex protein), and at 3,000 rpm to obtain a fraction rich in secondary metabolites, named LNP (latex non-protein). Specimens of H. contortus exposed to LNP, LP and PBS in the Adult Worm Motility Test (AWMT) were submitted to scanning (SEM) and transmission (TEM) electron microscopy to verify changes in their ultrastructure. Phytochemical tests in the LNP indicated the presence of phenols, steroids, alkaloids and cardenolides. High-Performance Liquid Chromatography (HPLC) characterized the presence of the compounds gallic acid and quercetin in the LNP. The protein content in the LP was 43.1 ± 1.1 mg/mL and 7.7 ± 0.3 mg/mL in LNP. In AWMT, LNP and LP inhibited the motility of 100% of the nematodes, with LNP being more effective than LP and ivermectin more effective than both (p <0.05). Cuticle changes were observed by SEM and TEM in nematodes treated with LP and LNP. Calotropis procera latex has anthelmintic effects against H. contortus, causing damage to its cuticle and other alterations in its ultrastructure.


Resumo Este estudo objetivou avaliar os efeitos anti-helmínticos e ultraestruturais do látex de Calotropis procera sobre Haemonchus contortus. Látex de C. procera foi centrifugado duas vezes à a 10.000xg e dialisado para obter uma fração rica em proteínas, denominada proteínas do látex (LP). E centrifugado e centrifugado a 3.000 rpm, para obter uma fração rica em metabólitos secundários, denominada LNP (látex não proteico). Espécimes de H. contortus expostos à LNP, LP e PBS no Teste de Motilidade dos Nematoides Adultos (TMNA) foram submetidos a microscopia eletrônica de varredura (MEV) e de transmissão (MET), para verificar alterações em sua ultraestrutura. Testes fitoquímicos em LNP indicaram a presença de fenóis, esteroides, alcaloides e cardenolídeos. A presença dos compostos ácido gálico e quercetina em LNP foi caracterizada por Cromatografia Líquida de Alta Eficiência (CLAE). O conteúdo de proteínas em LP foi de 43,1 ± 1,1 mg/mL e de 7,7 ± 0,3 mg/mL em LNP. No TMNA, LNP e LP inibiram a motilidade de 100% dos nematoides, sendo LNP mais eficaz que LP, e a ivermectina mais eficaz que ambos (p <0,05). Alterações na cutícula de nematoides tratados com LP e LNP foram observadas por MEV e MET. O látex de C. procera apresenta efeito anti-helmíntico sobre H. contortus, causando danos à sua cutícula e outras alterações em sua ultraestrutura.

Objective:To elucidate the key medicinal substances that cause the difference of efficacy between lotus leaf and lotus plumule, and to analyze their material basis of "homologous and different effect". Method:UPLC-Q-TOF-MS/MS technique was used to identify the main alkaloids in lotus leaf, lotus latex (juice in lotus petiole) and lotus plumule, chromatographic separation was achieved on a Waters XBridge C18 column (4.6 mm×150 mm, 5 μm), and gradient elution was performed with 0.1% ammonia aqueous solution (A)-acetonitrile (B) as mobile phase (0-13 min, 35%-60%B; 13-20 min, 60%-80%B; 20-20.1 min, 80%-95%B; 20.1-25 min, 95%B; 25-25.1min, 95%-35%B; 25.1-40 min, 35%B). Data acquisition was carried out in electrospray ionization (ESI) under the positive ion mode, the scanning range was m/z 100-1 000. Besides, the metabolic network of the main alkaloids was constructed. Result:A total of 5 alkaloids (N-nornuciferine, O-nornuciferine, anonaine, nuciferine and roemerine) were identified from lotus leaf, 6 alkaloids (nuciferine, norisoliensinine, 6-hydroxynorisoliensinine, liensinine, isoliensinine and neferine) in lotus latex, and 8 alkaloids (lotusine, norcoclaurine, N-methylcoclaurine, pronuciferine, armepavine, liensinine, isoliensinine and neferine) in lotus plumule. Also, the biosynthetic pathways of the terminal alkaloids of bisbenzylisoquinoline alkaloids (liensinine, isoliensinine and neferine) and aporphine alkaloids (N-nornuciferine, O-nornuciferine, anonaine, nuciferine and roemerine) was conducted. Conclusion:Five aporphine alkaloids in lotus leaf and three bisbenzylisoquinoline alkaloids in lotus plumule are the material basis for "homologous and different effect" of lotus leaf and lotus plumule. The metabolism of alkaloids in lotus leaf and lotus plumule is derived from the same compound of (S)-N-methylcoclaurine, and two types of alkaloids are synthesized through the action of two different enzymes. ABSTRACT

Objective To investigate the application value of Multi-Latex polygranular technique joint detection of kidney injury-related urinary microproteins in noninvasive diagnosis after renal transplantation. Methods Clinical data of 72 recipients undergoing renal transplantation were retrospectively analyzed. According to the level of serum creatinine (Scr), the recipients were divided into normal renal function group (group A, n=14), mild kidney injury (group B, n=37), and severe kidney injury group (group C, n=21). 20 healthy volunteers were selected as the healthy control group (HC group). The contents of urinary retinol binding protein (RBP), microalbumin (mAlb), IgG, transferrin (TRF), α1-microglobulin (MG), and β2-MG of subjects in each group were detected using the Multi-Latex polygranular technique. The correlation between urinary microproteins and Scr, blood urea nitrogen (BUN) was analyzed. The differences of urinary microproteins in each group were compared. And the diagnostic value of single and joint detection of urinary microproteins was evaluated. Results Six kinds of urinary microproteins in HC group and group A were significantly lower than those in group B and group C, and six kinds of urinary microproteins in group B were significantly lower than those in group C (all P < 0.01). Six kinds of urinary microproteins in renal transplant recipients were positively correlated with BUN. RBP, mAlb, α1-MG, and β2-MG were positively correlated with Scr. The correlations were statistically significant (P < 0.001-0.05). The diagnostic value of joint detection of urinary microproteins is better than the detection of single index, among which TRF+mAlb+RBP+α1-MG quadruple detection had the highest diagnostic value. Conclusions Six kinds of urinary microproteins can be used as specific indicators to reflect graft renal function. The polygranular technique can simultaneously detect its contents and achieve noninvasive diagnosis. The diagnosis based on TRF+mAlb+RBP+α1-MG quadruple detection is expected to further improve the noninvasive diagnosis system after renal transplantation.

ABSTRACT

Abstract Objective The present work aimed to evaluate the systemic effect of H. speciosa latex on bone neoformation. Methods For this, the latex was collected and diluted to 3% and 50%. A total of 28 Wistar rats were submitted to surgery to create a 5 mm diameter defect in the parietal bone. This experiment was conducted in 2 different periods: 1 and 2. For each period, the rats were divided into 3 groups: Control Group, Latex3 Group, and Latex50 Group, which received, respectively, daily administrations of 0.5 mL of distilled water, latex to 3% and latex to 50% by gavage, orally. The rats of periods 1 and 2 were euthanized, respectively, 15 and 30 days after the surgery, and the calvaria was collected. The results were analyzed using the ANOVA and Tukey tests; the significance level was 0.05. Results We show that, in each analyzed period, the experimental groups had the same amount of newly formed bone in the calvaria defect. Conclusion We conclude that daily and oral administrations of H. speciosa latex to 3% and to 50% over a period of 15 and 30 days does not contribute to the increase of the area of the newly formed bone in the calvaria defect.


Resumo Objetivo Este trabalho objetivou avaliar o efeito sistêmico do látex de H. speciosas obre a neoformação óssea. Métodos Para isso, o látex foi coletado e diluído a 3% e a 50%. Um total de 28 ratos Wistar foi submetido a cirurgia para a criação de um defeito de 5 mm de diâmetro no osso parietal. Esse experimento foi conduzido em dois períodos distintos: 1 e 2. Para cada período, os ratos foram divididos em 3 grupos: Grupo Controle, Grupo Látex3 e Grupo Látex50 que receberam, respectivamente, administrações diárias de 0,5 mL de água destilada, látex a 3% e látex a 50% por gavagem, via oral. Os ratos dos períodos 1 e 2 foram eutanasiados, respectivamente, 15 e 30 dias após a cirurgia e a calvária foi coletada. Os resultados foram analisados utilizando os testes ANOVA e Tukey; o nível de significância estabelecido foi 0,05. Resultados Mostramos que, em cada período analisado, os grupos experimentais tiveram a mesma quantidade de osso neoformado no defeito da calvária. Conclusão Portanto, concluímos que administrações diárias e orais do látex de H. speciosa a 3% e a 50% durante um período de 15 e 30 dias não contribui para o aumento da área do osso neoformado no defeito da calvária.

ABSTRACT

Background: Antimicrobial resistance is a devastating question that threatens health globally. The extensive, indiscriminate and unnecessary consumption of antibiotics for humans, as well as wildlife and in agriculture; lead to the development of notoriously resistant Staphylococcus aureus; through possession of mecA gene, encoded by modified Penicillin binding protein (PBP2a); being labeled “Methicillin resistant Staphylococcus aureus”. Conventional phenotypic techniques for MRSA detection rely on standardization of cultural characteristics. The latex agglutination method can be adopted as an accurate strategy for rapid detection of MRSA. Methodology: A total of 713 consecutive, non-duplicate isolates of Staphylococcus aureus were processed. Methicillin resistance was determined using cefoxitin (30 µg) by Kirby-Bauer method following Clinical and Laboratory Standards Institute (CLSI) guideline, latex agglutination method; and PCR for mecA gene. Results: The results showed that out of 713 Staphylococcus aureus isolates, 12.90% isolates were detected as MRSA due to resistance to cefoxitin. By latex agglutination method, 87 (94.50%) were positive for PBP2a; while on PCR mecA gene was detected only in 82 (89.10%) MRSA isolates. When assessed with PCR (gold standard) the sensitivity and diagnostic accuracy of latex agglutination was 100% and 94.57%, respectively. Conclusion: Latex agglutination test can be used as a prompt and reliable diagnostic technique for mecA gene detection in MRSA isolates, where molecular methods are limited. This can effectively minimize the misdiagnosis of resistant strains, and over/misuse of antibiotics.

Introdução: As patologias cutâneas representam mais de 35% das doenças relacionadas com o trabalho, afetando anualmente 1/1.000 trabalhadores. Objetivos: Caracterizar as dermatoses associadas ao trabalho em profissionais de um centro hospitalar e identificar possíveis agentes desencadeantes e fatores de suscetibilidade. Métodos: Estudo transversal com recolha da informação registrada no processo clínico eletrônico dos trabalhadores observados em exame de saúde periódico no período de um ano. Resultados: Nos 1.741 trabalhadores estudados, 3,56% tinham registro de dermatoses associadas ao trabalho, a maioria mulheres (76,85%). Dos profissionais com dermatoses, 34 (54,84%) tinham dermatite de contato irritativa, 17 (27,42%) urticária ao látex, seis (9,68%) dermatite de contato alérgica e cinco (8,06%) duas dermatoses concomitantes. Encontrou-se diferença estatisticamente significativa entre os vários grupos profissionais (p=0,008), sendo os auxiliares de enfermagem os mais afetados (5,11%). Verificou-se também maior prevalência (8,47%) em profissionais de blocos operatórios (p=0,001). Os principais agentes desencadeantes foram desinfetantes cutâneos, látex, luvas de nitrilo e contato prolongado com água (4,84%). Conclusões: Por tratar-se de um estudo baseado em registros clínicos, a informação está dependente da qualidade destes. Para além disso, os auxiliares de enfermagem foram a maioria representada na amostra no que se refere à população do centro hospitalar, o que limita a extrapolação dos resultados. Este estudo encontrou prevalência de dermatoses de somente 3,56%, o que talvez se deva à implementação de medidas preventivas. Os profissionais mais afetados foram os de blocos operatórios e os auxiliares de enfermagem. Os desinfetantes cutâneos foram os principais agentes apontados como desencadeantes.


Background: Skin diseases account for more than 35% of occupational diseases, affecting 1/1,000 workers annually. Objective: To characterize occupational dermatoses affecting hospital workers and identify possible triggers and susceptibility factors. Methods: Cross-sectional study in which we analyzed information extracted from electronic medical records of workers who performed periodic examinations in the course of one year. Results: About 3.56% of 1,741 included workers had a diagnosis of occupational dermatosis, being mainly women (76.85%). Thirty-four (54.84%) of the affected workers had irritant contact dermatitis, 17 (27.42%) latex allergy, 6 (9.68%) allergic dermatitis, and 5 (8.06%) two concomitant conditions. We found significant difference in prevalence as a function of occupational group (p=0.008), being highest for nursing assistants (5.11%). Prevalence was also higher for employees allocated to surgery departments (8.47%, p=0.001). Main triggers were skin disinfectants, latex, nitrile gloves, and prolonged contact with water (4.84%). Conclusion: The quality of the analyzed data depends on the quality of the analyzed medical records. Most subjects were nursing assistants, which fact hinders the generalization of the results. The prevalence of occupational dermatosis was just 3.56%, which might be explained by previously implemented preventive measures. The employees most frequently affected were those allocated to surgery departments and nursing assistants. Skin disinfectants were the most frequent triggers.

ABSTRACT

Cryptococcus spp. is a pathogenic fungus which is an increasingly important cause of infection, particularly in the immunocompromised hosts. Diagnosis of cryptococcosis in animals can be carried out by isolation of the fungus but this requires several days to detect and identify the organism. Detection of cryptococcal antigen by latex agglutination test and enzyme immunoassay in serum is a rapid and easy method for diagnosis of cryptococcosis. In the present study, a total of 142 blood samples were collected from apparently healthy (n=89) and diseased dogs (n=53) for diagnosis of cryptococcosis. Latex agglutination test and enzyme immunosorbent assay (EIA) were carried out for the detection of cryptococcal antigen in serum. Of the 142 serum samples tested, six samples tested positive by Latex agglutination test while one sample tested positive by EIA. The sample which was positive by EIA was also positive by Latex agglutination test. The serum samples of dogs that tested positive for cryptococcal antigen were obtained from dogs suffering from symptoms like bloody faeces and vomit, emesis, chronic ear infection and discharge. Based on our findings, we conclude that the latex agglutination test in combination with the enzyme immunoassay can be used for the diagnosis of Cryptococcosis in dogs

ABSTRACT

ABSTRACT CONTEXT: Adverse reactions associated with prick tests are rare but may be present as serious systemic reactions. CASE REPORT: A 38-year-old female nursing technician complained of three episodes of anaphylaxis in one year, all in the workplace. To investigate latex allergy, the patient underwent the prick test with latex, and immediately developed a rash, itchy skin, hoarseness, dyspnea and dry cough. Her condition improved promptly after appropriate measures were established for controlling her anaphylaxis. CONCLUSION: The skin test must be performed under medical supervision, since complications that can lead to life-threatening reactions, if support measures are not readily implemented, have been attributed to this test.

BACKGROUND/AIMS: Fecal calprotectin (Fcal) as well as the fecal immunochemical test (FIT) are useful biomarkers for detecting activity and mucosal healing in inflammatory bowel diseases. Here, we report the performance of simultaneous measurements of Fcal and FIT for ulcerative colitis (UC) patients using the newly-developed latex agglutination turbidimetric immunoassay (LATIA) system. METHODS: Fcal and hemoglobin were measured by the LATIA system in 152 UC patients who underwent colonoscopy. Fcal was also quantified with a conventional enzyme-linked immunosorbent assay (ELISA). Fecal markers were evaluated in conjunction with the mucosal status of UC, which was assessed via the Mayo endoscopic subscore (MES) classification. RESULTS: The LATIA system could quantify calprotectin and hemoglobin simultaneously with the same fecal samples within 10 minutes. The values of the Fcal-LATIA closely correlated with those of the Fcal-ELISA (Spearman rank correlation coefficient, r=0.84; P<0.0001). The values of Fcal for each assay and the FIT all significantly correlated with the MESs (Spearman rank correlation coefficient, Fcal-LATIA: r=0.58, Fcal-ELISA: r=0.55, and FIT: r=0.72). The mucosal healing predictability (determined by an MES of 0 alone) of the Fcal-LATIA, Fcal-ELISA, and FIT-LATIA with the cutoffs determined by receiver operating characteristic curve analysis was 0.79, 0.78, and 0.92 for sensitivity, respectively, and 0.78, 0.69, and 0.73 for specificity, respectively. CONCLUSIONS: The performance of the novel Fcal-LATIA was equivalent to that of the conventional Fcal assay. Simultaneous measurements with FITs would promote the clinical relevance of fecal biomarkers in UC.

BACKGROUND: This study aimed to investigate differences in neck pain, shoulder pain, and comfort between buckwheat and latex pillows in elderly sanatorium residents. METHODS: The crossover experimental study was performed with 30 elderly patients in a convalescent hospital for seniors. The buckwheat and latex pillows were used for 2 weeks, the 7 day was assigned as the washout between each one week intervention, respectively. Pre- and posttest visual analog scale (VAS) scores for neck and shoulder pains and the neck disability index (NDI) were investigated. Data were analyzed using paired-samples t test, and their correlations were analyzed using SPSS/WIN 21.0 (IBM Corp., New York, NY, USA). RESULTS: After the 2-week intervention, results showed that the use of buckwheat pillows decreased the VAS scores for neck and shoulder pains and NDI. Further, the use of latex pillows decreased the VAS score for neck pain and NDI, but the VAS score for shoulder pain showed no statistically significant differences (t=1.887, P=0.069). CONCLUSION: In conclusion, using latex and buckwheat pillows is the best option for reducing neck pain. Between the buckwheat and latex pillows, the buckwheat pillow is a better option for decreasing neck and shoulder pains.

ABSTRACT

Objectives@#To assess the diagnostic values of latex immunoturbidimetric assay (LIA) and particle immunofiltration assay (PIFA) for heparin-induced thrombocytopenia (HIT) .@*Methods@#Samples from 94 patients with suspected HIT from May 2016 to July 2018 in our hospital were prospectively analyzed by the two immunoassays. Their medical records and further follow-up data were also collected and analyzed by our hematologists to make the 4Ts scores and confirm the diagnosis of HIT, respectively. Performance characteristics of the two immunoassays were assessed, including sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) . Their post-test probabilities (PTP) were also calculated based on the 4Ts score.@*Results@#Among 94 cases, 15 (16.0%) had a positive HIT, including 6 of 37 (16.2%) with an intermediate, and 9 of 15 (60.0%) with a high 4Ts score. PIFA operating characteristics were: sensitivity 100.0% (15/15) , specificity 51.9% (41/80) , PPV 28.3% (15/53) , NPV 100.0% (41/41) . The positive PTP in intermediate and high 4Ts score group were 28.7% and 75.7%, respectively, while negative PTP were all 0. At manufacturers’ cutoffs, LIA operating characteristics were: sensitivity 66.7% (10/15) , specificity 94.9% (75/79) , PPV 71.4% (10/14) and NPV 93.8% (75/80) . The positive and negative PTP in intermediate 4Ts score group were 71.8% and 6.3%, while 95.2% and 34.4% in high 4Ts score group, respectively. Receiver operating characteristic (ROC) analysis manifested that LIA was preferable than PIFA, and combining the 2 assays together was significantly better than single test.@*Conclusions@#4Ts score is still an important tool for the diagnosis of HIT. Combining clinical score with heparin/PF4 antibody assay can increase the accuracy of confirming or excluding HIT. Although PIFA is inferior to LIA in the diagnostic value, its user friendliness and 100% NPV have major advantages. Combining the 2 assays together can achieve a higher diagnostic value.
